- The Chronic Disease Management (CDM) Nurse in the Diabetes Specialty Clinic utilizes the nursing process, critical thinking, and problem-solving skills to provide care to clients across the lifespan in the area of diabetes education and management.
- The CDM Nurse ensures the development, implementation, and ongoing administration of client care plans.
- The CDM Nurse assesses, coordinates, supervises, administers, and evaluates the provision of health services and healthcare resources.
- The CDM Nurse accepts responsibility for continuing competencies through ongoing professional development, including participation in education programs, research, and continuous quality activities.
- The CDM Nurse demonstrates the ability to assess client status accurately and effectively, and to take appropriate intervention measures that significantly impact the client's medical condition, safety, and recovery.
- The CDM Nurse may provide education and support in areas such as diabetes, weight management, cardiac risk reduction, secondary stroke prevention, and lifestyle management.
- In addition to a competitive rate of pay, Alberta Health Services currently offers a Remote Retention Allowance up to $3,000 annually, non-pensionable and payable on an hourly basis.
- As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities, and populations while ensuring their safety and well-being.
- You will utilize nursing processes, critical thinking, problem-solving, and decision-making, as well as teach, counsel, and advocate on behalf of patients and their families.
- You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance.
- You will provide safe, quality patient and family-centered care while reflecting the shared vision and values of AHS.
- Provisional Permit Holders will receive a specific pay rate, which will be adjusted upon registration as a Registered Nurse.

- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
- If no practice within the last five years, completion of specified free courses is mandatory.
- Experience in diabetes education and management across the lifespan is essential.
- Certified Diabetes Educator status required or in progress.
- Experience in facilitating chronic disease management education sessions.
- At least one year of relevant nursing experience preferred.
- Formal Education: RN required.
- A baccalaureate degree in nursing preferred; related experience considered.
- Certified Diabetes Educator recommended.
