The Clinical Navigator serves as a patient educator, advocate, and support person for patients who may benefit from health care navigation.

The Clinical Navigator collaborates with an interdisciplinary team that includes staff from LHSC and our community partners to facilitate a smooth and seamless transition of patients across the health care continuum.

This role uses the best evidence available to support patient and/or family education to promote self-management for optimal health for patients.

Regular Full-Time

Rate of Pay:
$43.04 per hour - $53.80 per hour

Hours of work: 37.5 hours per week


Qualifications:


  • Undergraduate Bachelor's Degree in a health related field
  • Mandatory current certificate of registration from a college recognized under the Regulated Health Professions Act or Social Work and Social Service Work Act
  • Project Management Certificate Preferred
  • Minimum of 5 years recent related clinical experience
  • Excellent communication skills written and verbal
  • Advanced knowledge of local and regional health care system
  • Advanced critical thinking and decisionmaking skills
  • Advanced People management skills
  • Ability to build collaborative partnerships
  • Proficient Microsoft Office skills
  • Demonstrated practice and commitment to the principles of patient and family centred care
  • Demonstrated practice and commitment to patient and staff safety
  • Demonstrated practice and commitment to LHSC's vision, purpose and values
  • Demonstrated ability to attend work on a regular basis
Before beginning employment at LHSC, all new hires will be required to:

  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis;
  • Provide documentation of the Tuberculosis skin testing (two step);
  • Provide proof of Health Canadaapproved COVID19 vaccination reflective of Ontario Health's current definition of being fully vaccinated
