Position Summary

  • The Program Manager, System Navigation, Float Team, Adult Allied Health, Cardiac Rehab is responsible for participating in program visioning, planning, evaluation, budget development, resource allocation, utilization management, risk management, and the promotion of service excellence and quality improvement. Accountable for the ongoing development of patient support resources and serves as the link between staff, physician leaders, acute care, After Hours Administrators, and Community Resources.
  • This role provides leadership, congruent with the Vision, Mission, Values and strategic objectives of the organization to Patient Flow Navigators, Acute Care Social Workers, Adult Allied Health team, and Cardiac Rehab and fosters partnerships with internal and external stakeholders to support efficient system navigation throughout the organization and community while promoting safe patient and family centered care.
  • PRIMARY RESPONSIBILITIES:
  • Fostering a work environment that promotes professional development and responsibility by:
  • Developing a motivated and cohesive team, supporting educational strategies (orientation and continuing education) and enhancing staff knowledge with respect to professional practices and patient care.
  • Promoting partnerships with key stakeholders.
  • Promoting a culture of trust and compassion and positive attitudes.
  • Conducting regular performance support discussions for all direct reports, identifying performance opportunities and successes which may include collaborative development of plans to support success.
  • Maintaining attendance support for all direct reports.
  • Encouraging positive staff morale.
  • Conducting regular unit/service meetings to ensure all direct reports have opportunity to receive or exchange information.
  • Coaching and mentoring of staff, students, individuals and teams.
  • Contributing to positive management/staff relationships.
  • Providing conflict management and counseling.
  • Managing unit safety programs.
  • Integrating and supporting students in all disciplines.
  • Working with the Scheduling Office to support staff scheduling, allocating leaves of absence, vacation schedule planning and authorizing overtime consistent with the operational needs of the unit(s) and within the scope of contractual requirements.
  • Ensuring staff is current with clinical practices and procedures in collaboration with clinical experts. Monitoring adherence to standards.
  • Managing change and fostering an environment which promotes personal accountability.
  • Ensuring that care requirements are met with appropriate competence and delivery.
  • Playing an active role in decisions concerning equipment and supplies to ensure appropriate quality.
  • Identifying and supporting resolution to barriers to patient flow.
  • Compiling, presenting and preparing data and executing action plans.

In conjunction with the Director:

  • Evaluating human resource needs collaboratively with the Manager, Human Resources, and direct care providers to select, retain and recruit individuals to meet the needs of the Program.
  • Establishing performance expectations, program goals, objectives and performance measures congruent with the Vision, Mission Values, Strategic Objectives and standards of the organization, collaboratively with colleagues and team members.
  • Monitoring internal and external customer service data as an indicator/ expectation of performance, ensuring prompt followup on customer complaints/concerns and utilizing data as opportunity for improvement or to celebrate success.
  • Measuring and comparing quality indicators, utilization, and program performance with established and corporate benchmarks in partnership with all stakeholders.
  • Monitoring fiscal utilization through annual, capital and operating planning processes and monthly
