The SMH Academic Family Health Team, which is part of the Unity Health Primary and Community Care Program is focused on providing comprehensive primary care to more than 45,000 patients.

The team services some of the lowest-income households in Down Town Toronto. In certain neighborhoods, there are high rates of low income and a higher burden of illness than the City of Toronto as a whole (e.g. St. James Town, Regent Park and Moss Park). The SMH AFHT has a vision to be global leaders in equity driven primary health care and advocacy.


The
Link Worker, Social Prescribing collaborates with an interdisciplinary team to complete social assessments of patients and connect patients and families to appropriate services.

The Link Worker engages in health teaching, making community connections, offering emotional support, accompanying persons to medical appointments, life skills teaching, and direct assistance with life tasks, as appropriate.

The role also requires supporting the team with delegated tasks that aid in the functioning and flow of the Social Prescribing program.


Qualifications Include:

  • Social Services education at the Community College level preferred, Health Promotion education an asset
  • Lived experience of social marginalization required (those with lived experience of being marginalized by poverty and discrimination are encouraged to apply)
  • Experience in a community based role, working with persons marginalized by poverty, discrimination, and/or homelessness
  • Strong knowledge of local community resources and past advocacy experience preferred
  • Demonstrates an understanding of the intersections between trauma, determinants of health and issues around homelessness, mental health and substance use.
  • Ability to work flexible hours as needed to meet the needs of patients
  • Ability to manage challenging encounters and deescalate situations, formal training is an asset.
  • Ability to work within an antioppression framework; understands and appreciates the diversity of our client and staff population and community.
  • Interest and/or experience in leading Peer Support Groups
  • Interest and/or experience in educational activities to promote the Link Worker role (both within the Health Centre and external community partners)
  • Comfortable working with clients in diverse external locations, and familiarity with St. James Town, Moss Park and Regent Park
  • Comfortable working from settings in the community, and partner agencies
  • Strong computer skills, specifically Microsoft Office, Outlook and a comfort using electronic charting records
  • Fluency in a second language an asset, particularly, those spoken by communities in the downtown east
