DESCRIPTION
The Patient Flow Coordinator will provide support to interprofessional teams ensuring timely flow of patients to the best possible location and guidance related to practice and policy in this regard.
This role involves the support and development of regional policies and procedures to improve access to appropriate and timely care.
In collaboration with the program leaders, Operations Managers, admitting team and as a member of the interprofessional team, the Patient Flow Coordinator is responsible for the effective and appropriate management of processes and resources throughout the organization.
The patient flow coordinator team manage various ministry reporting including but not limited to ALC data reporting and edits across the corporation.
PRIMARY RESPONSIBILITES & DUTIES:
Flow, Access and Discharge Plan Consultation
- While providing clinical/professional leadership within the organization, acts as a mentor, support and consultant to team members and external stakeholders in problem solving patient flow issues
- Facilitates patient processes to support effective patient flow from admission through to discharge, supporting effective resource utilization
- Working collaboratively with program leaders to manage multiple flow priorities including utilization of the surge protocol
- Revision and maintenance of discharge planning page content
- Support complex discharges for: ED, Inpatient, ALC avoidance
- Lead the ALC sign off process as well as ALC rounds
- Education related to discharge planning processes
- Development and implementation of patient flow and Alternate Level of Care guidelines to improve patient flow and access to care
- Managing confidential patient and organizational information
- Collaborate with community partners and external stakeholders to identify, plan, implement and evaluate strategies to enhance patient flow and access
- Use Regional communication to facilitate repatriations and swaps
- Maintenance of incoming/outgoing list of patient transfers
- Assist in Education as well as Repatriation pathway/policy review and review
- Keep Repatriation/interfacility transfer/ Swap data.
- Development and updating of policies, procedures and guidelines related to patient flow and access
- Participation in the development of regional and provincial policies, procedures and educational resources related to patient flow and access
- Planning, developing, implementing and evaluating discharge planning processes and practices
- Lead/ participate in the following:
- Various daily, weekly and monthly meetings, training and orientations.
- Daily WTIS and CCO data inputting
- Ensuring accuracy of wait time entries
- Fixing entries flagged for data quality management
- Monthly ALC compliance reports for CCO
- Acting as chair of the ALC working group
- Evaluating the quantity and quality of work performed re: ALC data and trends for the yearly performance reviews.
- Leading the monitoring, recording and analyzing of data and reporting trends to Senior Leadership
- PCS processing of ALC
- Monitoring of ALC list real time accurate
- Maintain coding competency via CCO guidelines
- Education to staff on ALC designation and process
- Various reporting including but not limited to datrelated to ALC, discharges, PCH, ED and TCU.

QUALIFICATIONS:

Undergraduate Degree in Nursing or a health related profession, or relevant education in case management or discharge planning:
Current certificate of registration from the College of Nurses of Ontario or applicable regulatory college:
Related post-graduate education preferred:
- 35 years of relevant clinical experience
- Experience working across the continuum of care is an asset (acute, rehabilitation, community care, long term care)
- Case management or discharge planning experience
- Experience in a tertiary care hospital preferred.
- Excellent critical thinking and problem solving skills
- Demonstrated effective communication with the healthcare teams, patients, family and community partners
- Demonstrated leadership and mentorship skills
- Demonstrated evidence of change management skills, negotiation and conflict resolution skills
- Proven ability to work independently and ability to work collaboratively within a team environment
- Excellent oral and written communication skills
- Skilled in use of information systems and technology (i.e. Wait Time Information System)
