Position Summary


Functioning as part of the coordinated service delivery team, the Outreach Worker works proactively with new and existing clients, offering a range of support and recovery services, within as existing service plan and on an as-needed basis.

The Outreach Worker is a relationship-builder and advocate and provides services in a non-judgemental and culturally appropriate manner, helping clients to build new skills and achieve their goals.


Qualifications

Education

  • Valid Drivers' license and access to a reliable vehicle
***
Service Delivery Skills, Expertise and Experience

  • Minimum of 2 years' experience working in community, preferably with program target population
  • Experience providing outreach with hard to reach populations
  • Must have experience working with a vulnerable population
  • Must have outreach and advocacy experience
  • Must have the knowledge and understanding of the Housing First model, and Assertive Community Treatment (ACT) methodology

Critical Skills

  • Proven knowledge and ability in relationshipbuilding, critical thinking, facilitation, decisionmaking, problemsolving and conflict resolution skills

Focus on Results

  • Demonstrated ability to prioritize and focus workrelated activities to effectively and efficiently achieve service goals and produce expected results/outcomes
