The Health Centre works with the most complex client population served by primary health care professionals within Waterloo Region.

Our highly skilled and diverse team works from a strength-based approach with individual clients as well as the broader community.

We collaborate with many partner agencies and organizations within Waterloo Region.

Our team consists of Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Chiropodists, Pharmacists, Psychiatrists, Health Promoters, Community Health Workers, Administrative Staff and Volunteers.

The Health Centre is currently recruiting for the following position:

CLIENT SERVICES ASSISTANT - OUTREACH

Position Summary


The primary focus of this position is to support KDCHC outreach teams that serve individuals who are homeless and precariously housed.

Often, these individuals are more complex due to mental health and addiction issues. This position works closely with outreach clients and provides administrative support to fixed and mobile primary care teams.

In addition, our outreach teams work with other partners of the Inner City Health Alliance that serve the same clients.


As a KDCHC outreach support position, the Client Assistant is responsible for a range of activities including providing social supports to outreach clients; providing general administrative support to the Outreach Team; providing administrative support for the Electronic Medical Record including supporting team members with data entry, pulling reports and troubleshooting; and liaising with other KDCHC primary care teams and services.


Specific Responsibilities
Outreach Tasks


  • Develop and maintain relationships with outreach clients
  • Support clients in filling out paperwork
  • Facilitate virtual appointments for clients with specialists
  • Advocate for clients with OW and ODSP
  • Arrange transportation for patients who are not mobile
  • Liaise and collaborate with Outreach and Primary Care Team members on shared clients
  • Connect clients to social supports where possible
Administrative Tasks


  • Answer calls and make client reminder calls
  • Maintain client charts including uploading faxes to charts, updating chart information, and transferring files
  • Make client appointments and support referrals i.e., to specialists
  • Follow up with health clinics and hospitals for test results, labs and consultations

Note:

KDCHC works on PS Suite EMR (PSS) however the Client Assistant also works with community partners using other EMRs.

This will require the Client Assistant to navigate more than one EMR system.


  • Participate in the development, implementation, monitoring and evaluation of health promotion/disease prevention programs for individuals and client populations as determined by the Health Centre.
  • Develop and monitor operating budgets for assigned projects/programs and ensure expenditures remain within budget.
  • As a Student Supervisor, provide orientation, ongoing support, coaching and monitoring to meet the requirements of the Health Centre and of the educational institution.
  • As a Volunteer Supervisor, provide support, training, monitoring and on-going communication as needed for the volunteer to successfully fulfill their position description.
  • Maintain a valid driver's license, access to a reliable personal vehicle and adequate insurance for work-related travel in accordance with the Health Centre's policies.
  • Work during both regular and extended hours (evenings/weekends) and in Outreach locations within Kitchener as determined by the Health Centre. A minimum of one evening per week will be required.
  • Participate in team and staff meetings and other meetings and committee work as appropriate to support both service delivery and organizational goals.
  • Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
