Responsibilities:
- Guides tenants through their individual wellness journeys by providing linkages to Northwood's continuum and the larger community resources and supporting their access to primary and continuing care services that optimizes their mental, social and physical well being
- Responsible for the coordination and management of all operations including the management of human, physical and financial resources for the Retirement Living Program at Northwood
- Provides leadership to the daily operations of the Assisted Living program
- Provides leadership to the daily operations of the Pauline Potter Health Centre including the Primary Care clinic
- Responsible for the promotion and marketing of the program and its services within Northwood, the community and surrounding area
- Responsible to ensure the provision of quality and safe services to promote optimal client and employee satisfaction
Tasks:
- Creates partnerships and relationships with tenants, families, friends, Northwood's programs and services, community resources; continuing care services, and organizations that support the goals of the Retirement Living program.
- Creates a culture of self referral among the tenant population to services provided at Northwood and in the community that support the tenant's social, mental and physical wellness.
- Works closely with the leasing agent, provides assessments for all Assisted Living and Supportive Living tenants and potential applicants when required
- Works with individual tenants develops a wellness plan and personal goals
- Liaises with health care professionals (e.g., care coordinators) in the access and arrangement of services for tenants
- Meets with families and tenants around transition and process to long term care.
- Assists tenants with the completion of governmental forms and/ or reading of correspondence
- Performs other related duties as required
Qualifications:
- Current Licensed Practical Nurse Designation
- Minimum 5 years supervisory experience
- Experience in long term care, assisted living and/or homecare setting considered required
- Leadership Development training an asset
- Strong knowledge of Continuing Care and Communal living
- Computer proficiency and working knowledge of word processing, database, spreadsheet
- Experience in business operations, housing, senior housing, navigating community based programming i.e. healthcare, wellness.
- Experience dealing directly with individuals who are experiencing health and/or wellness issues
Salary:
As per Northwood Scale
Hours:80 hours bi-weekly (8 hour shifts)
Start Date:
Immediately
