**The Opportunity

The Subacute Access Coordinator is the facilitator/liaison that focuses on screening Strata Pathways Profiles for admission to the subacute programs RCTP, ERCTP, and ALC programs across Carewest Glenmore Park, Sarcee, and Dr.

Vernon Fanning.

The incumbent of this position will coordinate, review, assess, and collaborate with AHS acute care units, Transition Services and Carewest Subacute programs RCTP, ERCTP and ALC.

The overall objective of this role is ensuring a consistent approach to screening all clients for the subacute program, timely and efficient review of the profiles, smooth transition of care from acute care to subacute to ensure quality of care and decrease wait times to assist with acute care capacity.

The Subacute Access Coordinator works collaboratively with acute care nursing, AHS transition services, Carewest Client Service Manager, Clinical Support Coordinator, and subacute team leaders.

The Subacute Access Coordinator will triage referrals and seek to optimize all referrals within the subacute programs to ensure the client is matched with the most appropriate program to meet the client current needs.

Qualifications & Experience:

Qualifications: Health Care Degree, Designation and/or Registration. Healthcare Registration in good standing.


Preferred:
Indigenous Awareness and Sensitivity training or certificate.

Required Organizational Practices (ethics governance documents, policies and procedures, AHSecure:
collect it -Protect it). AHS AIW training (fundamentals) or other continuous improvement methodology. PROSCI change management modules or certification.

Preferred: Progressive leadership experience in a healthcare environment including leadership of teams of significant size, complexity, and diversity. Continuing care and accreditation experience are assets
