About the Opportunity:


The East Coast Forensic Psychiatric Hospital (ECFPH) Rehabilitation Service has a provincial mandate to work in partnership with clients and assist them to move toward recovery by obtaining the skills and supports they need to be successful and satisfied living in the community of their choices, while assuring public safety.

This position is responsible to work in partnership with the client to assess and establish rehabilitation interventions appropriate for the client that would foster smooth community integration.


The Coordinator works in partnership with the client/family and the interdisciplinary team to assess the client's needs and develop a person-centered care plan to address the needs.

This position also acts as a liaison between the client and care team to ensure the needs of the clients are identified and addressed.

The coordinator will ensure the continuity of clients care into the community by offering case management including assessment, monitoring and management of risk factors of clients living in the community who are under the Criminal Code Review Board.


About You:


  • Bachelor's Degree in a health professional discipline (e.g. Social Work, Occupational Therapy, Nursing)
  • License/Registration with relevant discipline
  • 2 years related experience required
  • Demonstrated understanding and knowledge of Mental Health Case Management and Recovery
  • Demonstrated understanding and functional knowledge of the CCRB mandate and risk management.
  • Program planning, development, implementation, monitoring and evaluation
  • Demonstrated Leadership skills and knowledge of LEADS in a caring Environment
  • Experience working in a Psychiatric Forensic setting an asset.
  • Experience in Mental Health Case Management Service delivery and Psychosocial Rehabilitation.
  • Demonstrated strong interpersonal skills including ability to work sensitively and effectively with clients, families, various professional and community agencies and partners in a collaborative team environment
  • Knowledge and experience in Risk management as asset.
  • Understanding of the mandate of Criminal Code Review Board.
  • Working knowledge of Section 16 and 672 of the Criminal Code of Canada
  • Understanding of Mental Health Diagnoses
  • Good awareness of community resources (province wide)
  • Effective leadership skills (communication, collaboration, negotiation, etc.)
  • Knowledge of recovery, PSR and Case management principles
  • Competencies in other languages an asset, French preferred
  • Leads in a caring environment

Hours:


  • Permanent Full Time position; 75 hours biweekly
  • Travel will be required

Compensation and Incentives:

$ $42.68 hourly
