Job Summary

The Social Worker is an integral member of the clinical care team providing direct support to the clinic's Geriatricians and Nurse Practitioners.

The Social Worker will provide direct patient assessment, consultation and counselling for complex seniors in an interprofessional outpatient clinic setting while developing and maintaining partnerships with our community partners.

Qualifications:


  • Masters' Degree in Social Work from an accredited university required.
  • Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), with no terms, conditions, or limitations required.
  • Minimum 2 years of experience working with the geriatric population;
  • MOCA certification required, or willing to obtain upon hiring

Primary Responsibilities

  • Utilizes high level of clinical judgement, critical thinking and expertise in senior care to perform comprehensive chart review and preliminary assessment/case history;
  • Acts as a liaison between the patient, consultants, resources and the referral source. This may include crisis intervention between family members, hospitals, agency service providers, police and patients;
  • Provides patient and family centered assessment and management of a range of individual needs including: social dynamics, elder abuse, adjustment counselling, and illness understanding;
  • Facilitates linkages with primary providers and community supports, as needed
  • Documents all relevant information, as well as any ongoing contacts with patient/family/professionals which impact patient needs or intervention by the care team;
  • Participates in the identification of issues and formulation/revision of departmental policies and procedures relevant to clinical operations.
