WellFort is seeking a Community Dietitian to join our team

Up to 0.7 FTE contract up to March 31, 2024 (part-time)

Community Dietitian - High Priority Communities Strategy (CD-HPCS)
The Community Dietitian (CD-HPCS) develops and provides nutrition education programs for individuals, families, groups and communities.

The CD-HPCS works as a part of the inter-disciplinary team functioning both independently and with other staff in clinical and outreach activities.


While the proportion of time allocated to different functions may vary according to service needs, the CD-HPCS's role encompasses a combination of direct care, health promotion, education and outreach, advocacy, community development, program development and evaluation.

Responsibilities:


  • Develop, implement and evaluate individual nutrition care plans according to the parameters of client's health, literacy level, income and relevant cultural and psychosocial factors
  • Ensure continuity of care by maintaining uptodate medical records, monitoring clients on a regular basis, and participating in case conferences with other members of health care team, as required
  • Participating in the development, implementation, monitoring and evaluation of treatment, outreach and education, counseling, and health promotion services for individuals, families, and the community.
  • Support the advocacy efforts of clients, agencies and policy makers to reduce barriers for clients and the broader community.
  • Maintains clear, accurate and timely written and electronic documentation including EMR, requisitions, reports, encounters and other information as required by the health centre, MOH, other regulatory bodies.
  • Maintains an uptodate knowledge of government guidelines, acts and legislation affecting client care.
  • Develops and maintain a flexible work schedule that accommodates the needs of the clients, community and the Centre
  • Provide care delivery at any of the office sites as program needs dictate.
  • Other duties as assigned.

Qualifications:


  • Undergraduate degree in food and nutrition from a recognized university.
  • Completion of a Canadian Dietetic Association accredited Dietetic Internship.
  • Registration with the College of Dietitians of Ontario and maintains current membership in the Canadian and Ontario Dietetic Associations.
  • A minimum five years of clinical counseling and community nutrition experience in a variety of primary care settings, one of which must be community health.
  • Demonstrated commitment to best practices, ongoing learning, and client centered care.
  • Experience working with communities and individuals who face barriers accessing traditional health care institutions due to poverty, discrimination, mental health and substance use issues.
  • An understanding of and critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health.
  • Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level.
  • Experience working with confidential and time sensitive documents and information.
  • Excellent interpersonal skills and the ability to multi task in a fastpaced environment.
  • Experience working in a value based nonprofit or social services organization.
  • Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.
  • Ability to speak languages relevant to the residents of the organization's community is an asset.
  • Consistent access to a vehicle for responsive care delivery at any care delivery location.

Initial Hiring Salary Range:
$ $35.11/hour inclusive of vacation pay (paid time off)
