The Opportunity:
The Middle Year's Worker is responsible for implementing and planning programs and services for youth.

The worker is also responsible for referring families to the appropriate services throughout the community, working with families, and professionals in the community to ensure children 6-12 years of age are fully supported.**- Directly assist communities to increase their capacity to identify, and address child and youth development.

Collaborate with individuals and communities to identify and provide programs and delivery methods that are acceptable to them and responsive to their needs.
Establish/maintain quality community awareness and support and referral systems for the programs.
Develop and maintains community referral sources
Provide multiple Equine Therapy programs to CSFS communities.
Create ongoing programming for the Middle Years program
Provide Yinka Dene Camps to various communities.
Attend and actively participate in Integrated Care team Meetings and case management meetings.
Transport children to programming
Act as a liaison with other health and social service professionals
Keep ongoing, accurate client records
Collects data for biyearly reports

What You Will Bring to The Role:

Social Work Certificate, Diploma, or the equivalent:
