Reporting to the Manager of Specialized Services,
the incumbent will be responsible for the following functions:
- Knowledge and understanding of client centred mental health and addictions interventions and practice methods
- Conducting and completing detailed interRAI Assessment and synoptic report for individuals referred;
- Will coordinated with the MHEART Nurse as needed regarding medication and potential side effects as well as the dispensing of medication where required
- Providing crisis prevention and intervention planning with the intention of preventing a relapse and/or hospitalization
- Compiling relevant case histories and gathering collateral information;
- Participating in Case Disposition meetings as needed to determine outcomes for individuals referred to the Agency;
- Exploring and linking individuals to external community resources to meet mental or physical health needs;
- Communicating and coordinating relevant information with agency staff and police;
- Responding and coordinating care with Bluewater Health Psychiatry, Emergency Department Physicians and Psychiatric Assessment Nurses.
- Provide community presentations as needed
Qualifications:
Education required:
- Postsecondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the C.NO. Bachelor's Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or a postsecondary degree with a relevant professional registration
- a valid Ontario Driver's Licence and access to a vehicle are required
Hours of Work:
Week 1: 12:00 pm - 10:00 pm, Monday - Thursday
Week 2: 7:00 am - 5:00 pm, Tuesday - Friday
Salary:
Commensurate with experience within the range: $66,923-$81,015 inclusive of HOOPP
