Position Summary

Under the supervision of the Resident Care Manager, the Social Worker is responsible for supporting residents and families within our sites.

As an integral member of the inter-disciplinary team, the Social Worker will provide the assessment, evaluation; care planning, care delivery, education and information sharing for residents and families.


  • Collaborates with the healthcare team to provide meaningful holistic care and support for residents and families ensuring choice, dignity, privacy, respect and individuality.
  • Responsible for the continuous improvement of the care and support program for residents and their families.
  • Builds and maintains therapeutic relationships and provides counseling and crisis intervention
  • Evaluates clinical and program processes, care delivery and outcomes to ensure the needs of residents and families are identified and addressed
  • Assists in building community capacity in the external community
