Project Manager, Seniors' at Risk in Hamilton Collaborative**
A Collaboration between Alzheimer Society, St. Matthew's House and Good Shepherd

(Contract - Up to 1 year)


Seniors' at Risk in Hamilton (SaRiH) is a newly established model of service delivery that responds to the needs of vulnerable and marginalized older adults in Hamilton through a unique collaboration between
_Alzheimer Society, St.

Matthew's House and Good Shepherd_.

SaRiH is founded on a commitment to offering a holistic, integrated approach to supporting older adults at risk due to a broad range of health determinants.


SaRiH is currently seeking a Project Manager to support the evolution, execution and evaluation of Seniors at Risk in Hamilton (SaRiH).

Working closely with member agencies, the Project Manager will support the development of structures and processes that will drive innovation, excellence in service delivery, rich stakeholder engagement and enhanced capacity in responding to the needs of at-risk seniors in this community.

The Project Manager will assist member agencies in building this innovated collaborative through the following activities:


Deliverables:


  • Supporting partners in the design and execution of service elements including common referral and consent, shared policy and procedures and crossagency training and capacity building
  • Facilitating consultation and decisions related to shared technology and data management
  • Supporting communication and engagement with stakeholder groups and broader community
  • Facilitating engagement and coordination with key stakeholders and allied service providers in health and human services to promote innovation, coordination and collaboration in the delivery of services to vulnerable older adults.
  • Complete a developmental evaluation of SaRiH implementation and manage report back to Ontario Health and community partners/stakeholders

Education and Experience:


  • Minimum of 4 years of project delivery experience, preferred
  • Minimum of a bachelor degree in a health or social system related field OR demonstrate similar level of education and experience with the key areas of accountability
  • Hold a combination of certification and/or experience in delivering the full project management lifecycle
  • Understanding of the social service and healthcare sector and experience in either of these environment
  • Demonstrated experience in program planning and implementation, managing relationships and system transformation
  • Exemplary communication, interpersonal and group facilitation skills, as well as a demonstrated ability to collaborate and cultivate productive relationships with a diverse range of key stakeholders.
  • Commitment to working within an integrated intersectional, antiracist/antioppressive framework
  • Verbal and written skills in a language in addition to English an asset

Knowledge and Skills:


  • Excellent priority setting skills with the flexibility and adaptability to respond to shifting business.
  • Excellent problemsolving, critical thinking, deductive reasoning, inductive reasoning, and analytical skills
  • High professional standards and strong customer experience mentality with an energetic, positive attitude to build partnerships and relationships
  • Ability to work independently, manage multiple tasks and deadlines, and solve challenging problems in a fastpaced, environment while also working collaboratively in a team environment
  • Strong knowledge translation and mobilization skills, ability to extract meaning from technical/clinical details
  • Demonstrated experience with the organization, implementation and ongoing operations of multifaceted programs is preferred
  • Forwardthinking team player who understands the value of collaboration across teams, and integration with the business
  • Selfmotivated, a selfstarter and have highenergy
  • Excellent proficiency with use of communication tools (i.e. MS Project, MS Office / O365 suite)
