This person will hold a large knowledge of the healthcare industry and enjoy the flexibility a small businesses has to offer.

Roles and Responsibilities Client Care

Establish and monitor a care client plans to ensure the level and quality of care is met
Ensure progress notes are complete and maintained in an orderly fashion.
Ensure client documentation is complete as per guidelines.
Discuss client needs with families, physicians, and other health care professionals.
Liaise with families to ensure agreements and onboarding documents are signed prior to service.
Demonstrate the ability to interact effectively within the team, with residents and clients and families as well as external service resources.

Qualifications and Education Requirements

RPN/RN Diploma/Certificate is preferred
College/University degree in healthcare is an asset
2/3 years job related experience as a Scheduler/Coordinator in a healthcare setting
Efficient verbal and written communication skills
Ability to multi task and problem solve independently in a fast paced environment
When off duty, be prepared to consult or give advice over the telephone and/or be on call for emergencies.
Work hours may vary with some evening and weekend work
