Qualifications:


  • Certificate in health education/promotion, human services and/or other related field
  • Two to five years experience in community programming
  • Must be able to work under mínimal supervision
  • Valid Drivers license
  • Excellent organizational, interpersonal and communication skills
  • Customer Service
  • Knowledge of Microsoft Office (including Excel) and Accounting Software
  • Policies and procedures
  • Aware of programs offered in the Community and resources
  • Aware of process to complete an interview
  • Demonstrated knowledge in computerization, including MS Office including Excel and Accounting Software
  • Excellent keyboarding skills
  • Demonstrated strong verbal and written communication skills
  • Skills: Confidentiality, attention to detail, accuracy, organization, maturity, flexibility, ability to work well under pressure, analytical, strong team player, adaptable to change, ability to set and adjust priorities frequently
  • Demonstrated ability to attend work on a regular and consistent basis.

Responsibilities:


  • Engage tenants of the Lakeview Manor, Sealloah Breagh Ion ORKeltic View Manor and Capstick Manor in the Cluster Care pilot.
  • Complete participant registration forms for the pilot
  • Implement recreational programming for participants using volunteers, community groups, and CCAs
  • Connect participants with VCHSS Community Connections volunteer programs such as prescription pick up, item removal to transfer station, social visits
  • Provide navigation to tenants for community resources
  • Refer tenants to Continuing Care services and programs where appropriate
  • Refer tenants to Nova Scotia Provincial Housing Agency where appropriate
  • Responsible for weekly programming schedules
  • Assist with Age Care Technologies interviews
  • Liaise with VCHSS Community Connections Administrative Assistant to connect volunteers with tenants as needed
  • Liaise with VCHSS Schedulers and LPN Supervisor regarding CCA schedules in the respective buildings
  • Report to VCHSS Community Connections Supervisor
  • Performs other accounting duties as required

Hours of Work: 70 hours bi-weekly
