about the role:


The IPC System Navigator will provide support to clients on the Health Bus and complete follow-up at Sherbourne Health and/or in the community by providing trauma informed case management support to clients.


The IPC System Navigator liaises with multiple stakeholders, including clients, primary care providers, the Acute Respite Care program, and local community organizations to facilitate care coordination and case management.


Day to day responsibilities:


Case Management Services

  • Works on the Health Bus with homeless and precariously housed individuals who have mental health and/or substance use issues
  • Followup with clients serviced on the Health Bus at Sherbourne Health and in the community
  • Operates the Health Bus on a regular basis, driving the health bus to weekly stops, maintenance appointments, etc.
  • Provides case management support to other dropin programs at Sherbourne Health that support homeless and/or precariously housed individuals as well as individuals who have mental health and/or substance use issues
  • Assesses client status and service needs
  • Provides crisis intervention, systems navigation, accompaniments with clients and makes appropriate referrals
  • Works closely with the client, members of the IPC team and community partners to develop and/or collaborate on the development of coordinated care plans
  • Provides case management support, utilizing a variety of techniques and approaches that are traumainformed, strengthbased, culturally sensitive and appropriate to client's needs
  • Attends and arranges case coordination meetings as required
  • Facilitates client referrals for internal or external community services
  • Distributes harm reduction supplies and naloxone as required

Health Promotion, Education and Advocacy

  • Provides appropriate client advocacy services as directed by the client
  • Collaborates and consults with multidisciplinary staff, partnering organizations and broader community to facilitate comprehensive care
  • Participates in outreach activities
  • Engages in evidencedbased practice and ongoing learning
  • Advocates for individual client needs to ensure access to effective care, followup and resources from both an individual and systemic level
  • Facilitates knowledge translation by educating colleagues on topics related to client care
  • Initiates and/or participates in systemic advocacy activities directed at achieving health equity by improving the social determinants of health and eliminating stigma

Administrative Duties

  • Contributes to overall policy and program directions
  • Participates in ongoing evaluation of programs and service delivery.
