Position Description:


The Patient Flow Coordinator will be responsible for the delivery of patient care by providing a high level of support and assistance to patients, families and the team in the Healthy Living Clinic.

They will support the operation of the clinic by ensuring patients and families flow smoothly and efficiently through their visit in the clinic in a proactive and organized manner, facilitate clinic schedules and patient bookings, support program initiatives and quality improvement projects, ensuring seamless program operations.

Patients within the Healthy Living Clinic enter a 2-year intensive program focused on a multidiciplinary approach to meeting patient needs.

As the Patient Flow Coordinator you will play in integral in role in the coordination of all clinics, services, groups and patient movement throughout the program, as well as the day to day operations of the clinic.


Here's What You'll Get To Do

  • Greet patients and families and orient them to the clinic
  • Support the flow of patients and families starting from registration through to the end of their visit
  • Work closely with the clinical team to ensure smooth and seamless daily operations of the clinic
  • Support clinical staff in organizing and storing materials that are utilized in the clinic
  • Facilitate communication flow via telephone, paging, and computer access
  • Book and/or reschedule appointments and group sessions as necessary and inform necessary people of these changes
  • Collate clinical data and relay this information in a precise and timely manner to colleagues within and outside of the organization
  • Support the registration and flow of patients within the clinic
  • Maintain an efficient and tidy work environment
  • Initiate and assist in the coordination of family/patient and health care professional education sessions and events
  • Communicate with interprofessional team members within and outside of the organization to coordinate the booking visits
  • Track and faciliate patient movement through the Healthy Living Clinic, clearly communicating to team and families expectations
  • Support a centralized repository of Healthy Living Clinic documents and program guides that are accessible by all team members

Here's What You'll Need

  • Bachelor's Degree in a related field (Medical Administration, Business Administration, Health Administration) preferred
  • Minimum of 3 years clerical or administrative experience in a health care setting. Experience in a hospital environment is preferred
  • Scheduling experience is essential and medical terminology is an asset
  • Accurate keyboarding skills and strong working knowledge of Word, Excel, PowerPoint, and Outlook
  • Experience with Epic is an asset
  • Experience in surgical patient populations is an asset
  • Excellent problem solving skills that demonstrate an ability to anticipate potential issues and make suggestions
  • Strong interpersonal, communication and organizational skills with both fellow colleagues and patients
  • Ability to remain focused and prioritize tasks in a busy and high stress environment
  • Dependability and good attendance
  • A commitment to understanding and aiding in the pursuit of equity, diversity & inclusion
