Housing Stability Worker, Street Level Women at Risk
Full-Time, Permanent

Grounded in London's housing stability services system, the Street Level Woman t Risk (SLWAR) plan is aimed at supporting housing stability, long term health, wellbeing and belonging for women experiencing homelessness.


The model is framed in service collaboration to help provide housing with in home support, and a system of wrap supports that are participant lead and based on the goals each woman has for herself.

WHAT WE'RE OFFERING:

What you'll be making - $ $25.71 per hour, rate based on proof of comparable, previous experience

When you'll work:

  • 70 hours over twoweek period, including days, evenings, weekends and public holidays

  • Immediate enrolment in the Healthcare of Ontario Pension Plan (HOOPP)

HOW YOU WILL BE MAKING A DIFFERENCE:

  • Providing a welcoming, safe and trusting environment for participants
  • Providing intensive case management including regular, continuous contact with the participant
  • Brokering and advocating with other system elements, accompany participants to appointments, provide proactive crisis planning, organize meaningful daily activity opportunities and liaise with Ontario Works and Ontario Disability Support Program
  • Connecting the participant with appropriate services or supports based on their needs, including physical healthcare, mental healthcare, addiction services, trauma services, vocational assessment and training, employment, volunteer and socialrecreational opportunities
  • Preparing and planning for "homelessness proofing" and rehousing. Includes recognizing and averting indicators that may destabilize a participant's housing and focus on what worked when rehousing a participant
  • Maintaining a regular caseload of an average of 13 participants at any time

WHAT YOU'LL NEED TO APPLY:

_We require the following qualifications:
_


  • Postsecondary education in a relevant field of study e.g. social work, psychology, or a related discipline
  • Thorough understanding of harm reduction theory and demonstrated ability to implement harm reduction strategies
  • Extensive knowledge of community resources, service & support programs, and barriers participants face in accessing same
  • Working knowledge of a variety of treatment, intervention, and support strategies
  • Experience with ongoing evaluation of complex case management or treatment plans, engaging with the participant and other resources in adjusting the plan, as appropriate
  • Computer literacy
  • Ability to assess level of risk in regards to participants
  • Valid Driver's License, adequate insurance and access to a vehicle to carry out job duties
  • Current, clear Police Vulnerable Sector Check (PVSC)
  • First Aid & CPR
  • Demonstrated ability to attend work regularly

We prefer the following qualifications:
_*- Minimum of 3-5 years' experience working in mental health, preferred

  • Training and experience using Service Prioritization Decision Assistance Tool (SPDAT) and the Homeless Individuals and Families Information System (HIFIS)
