Detailed Overview:
In accordance with the Mission and Values of Fraser Health, the Transportation Attendant/Health Care Assistant works collaboratively and in partnership with the interdisciplinary team in the provision of person-centered care to diverse populations promoting resident/client choices, individualized routines, resident/client and family involvement in care decisions and continuity and consistency of care.
Operates a passenger transportation vehicle with seating capacities of up to 10 or between 11 and 25 passengers including the driver while carrying one or more passengers.
Performs Health Care Assistant duties by following an established, individualized care plan, assisting residents/clients in activities of daily living, participates in organizing/implementing established leisure activities, encouraging resident/client participation within their abilities and informs residents/clients, families and volunteers of daily plans.
Makes tea, coffee, toast, sandwiches and assists residents/clients who require help with their dining experience.Responsibilities:
- Transports either up to 10 or between 11 and 25 residents, staff, volunteers and family members, including driver, on scheduled outings. Assists in embarking and disembarking passengers utilizing all safety routines and procedures. Provides direction to staff in matters related to bus safety.
- Assists staff with outings by transporting residents in wheelchairs and loading and unloading activity supplies.
- Performs daily, weekly, monthly and yearly routine inspections/maintenance duties on the wheelchair bus and bus equipment such as lift, tiedown straps and emergency equipment.
- Assists in scheduling and coordinating bus outings as requested and maintains records in accordance with established procedures.
- Following an established, individualized care plan, provides personcentered care by assisting residents/clients in activities of daily living. Encourages and assists as necessary with toileting, bathing, skin care, grooming, dressing, personal laundry and mobilizing residents/clients.
- Participates, as a member of and in collaboration with the interdisciplinary team, in the organization and implementation of established leisure activities as outlined in the care plan; encourages resident/client participation, within the resident's/client's abilities and informs residents/clients, families and volunteers of daily plans.
- Demonstrates actions and movements to residents/clients and encourages the involvement of family members and volunteers in specific, appropriate 1:1 activities with individual residents/clients as per the care plan.
