IT & EMR Coordinator


The Southlake Academic Family Health Team is seeking a full-time information, technology and electronic medical records coordinator to support our large primary care practice.


Job Responsibilities:

  • Oversee IT and telecommunication platforms, including, but not limited to our electronic medical records (EMR), patient engagement tools, website and communication systems.
  • Provide leadership for the identification, investigation and implementation of technology and/or innovation solutions that may improve patient care, staff or physician productivity or data collection and ensure the appropriate key stakeholders are engaged throughout this process.
  • Creation of Custom Forms/Handouts for our EMR.
  • Assist with EMR improvements, data collection and analysis.
  • Assist the Quality Improvement and Program Manager to generate and disburse regular reports on identified clinical quality indicator results for quality improvement activities utilizing flow charts, graphs and diagrams;
  • Assist with EMR data extraction, searches and standardization of clinical terms which would include forms, dashboards, cleaning of data, standardization, etc.;
Required Experience

  • Post secondary school degree/diploma in a relevant field.
  • Strong verbal and written communication.
  • Demonstrated high competency in computers and other IT platforms; this includes knowledge in both computer hardware and software programs.
  • Formalized training and experience with creating custom forms for electronic medical records preferred.
  • Previous experience working with computerized documentation systems, in particular data collection.
  • Previous administrative experience in a health care environment, particularly Family Health Teams, is preferred.
  • Previous experience working with regional and provincial databases including but not limited to, Health Quality Ontario, Cancer Care Ontario and Connecting Ontario, is preferred.
  • Experience and/or knowledge with TELUS Practice Solutions, is highly preferred.
