About the Opportunity:


Please note parking is FREE at ECFH and we have a café on site for staff. This is located at Burnside in Dartmouth.**The Coordinator works in partnership with the client/family and the interdisciplinary team to assess the client's needs and develop a person-centered care plan to address the needs. This position also acts as a liaison between the client and care team to ensure the needs of the clients are identified and addressed. The coordinator will ensure the continuity of clients care into the community by offering case management including assessment, monitoring and management of risk factors of clients living in the community who are under the Criminal Code Review Board. The position requires a high degree of both multidisciplinary and independent work for an individual who likes diversity in their work environment, flexibility in scheduling and being part of a team.


About You:

We would love to hear from you if you have the following:

  • Bachelor's Degree in a health professional discipline (e.g. Social Work, Occupational Therapy, Nursing)
  • License/Registration with relevant discipline
  • 2 years related experience is preferred
  • Demonstrated understanding and knowledge of Case Management, Recovery, Psychosocial Rehabilitation. Risk management and knowledge of the CCRB would be preferred
  • Program planning, development, implementation, monitoring and evaluation
  • Past experience working in a forensic environment would be considered a definite asset.
  • Demonstrated strong interpersonal skills including ability to work sensitively and effectively with clients, families, various professional and community agencies and partners in a collaborative team environment.

Hours:


  • Long-Assignment/ Full-Time (100%) / 75 Hours Biweekly
  • Start as soon as possible, with an approximate end date of March 202
  • There is an opportunity to work from home one day a week once orientation is completed.

Compensation and Incentives:

$ $42.68 Hourly
