Wyndham House offers a wide range of programs and supports to at-risk and homeless youth. We are currently seeking an Intensive Case Manager, Supportive Housing Program.


JOB TITLE:
Intensive Case Manager, Supportive Housing Program


POSITION LEVEL:
Experienced; non-manager


POSITION TYPE:
Full Time salary; 3-month probationary period


HOURS:
40 hours/week; Tuesday-Saturday 8:30 a.m. to 4:30 p.m.; with some flexibility, including 1 afternoon shift/week


The Intensive Case Manager, Supportive Housing Program provides case management, including service coordination, advocacy, recreation support and life skills development support to a case load of clients living in the Supportive Housing Program.


JOB SUMMARY

  • Support life skills development
  • Coordinate clients' access to community services
  • Collaborate with community partners to provide wraparound support
  • Provide or support programming related to skills development, recreation and therapeutic goals
  • Provide traumainformed and clientcentered care

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Design and implement programming to facilitate life skills development
  • Identify community services and resources that are appropriate for clients' needs, coordinating referrals, intakes, appointments, and ongoing access to services
  • Coordinate with schools to build appropriate school programs and supports, including intake at appropriate schools, IEPs, coop and SAL options, adjusted schedules, and school transfers
  • Coordinate case conferences with involved community partners
  • Support youth through their involvement with the Youth Justice System, including court, lawyers, mental health diversions, and probation
  • Develop relationships with clients to support emotion regulation, harm reduction, and life skills development
  • Liaise with staff to coordinate care and routines in Supportive Housing Programs and support with behaviour management and treatment goals.
QUALIFICATIONS / REQUIREMENTS

  • Post-Secondary education in a Social Services or related field (BSW preferred)
  • 2+ year's experience working with youth with complex needs
  • Sound knowledge of the barriers facing streetinvolved youth and commitment to the principles of housingfirst
  • Skills, knowledge, and attitude consistent with traumainformed clientcentered advocacy and care
  • Demonstrated knowledge of local social services and the processes by which to engage them
  • Demonstrated understanding and sensitivity to social and cultural diversity, including ability to support clients' decisions without judgement or bias
  • Strong organizational, communication and interpersonal, and crisis intervention skills
  • Strong ability to problem solve and work independently
  • Background facilitating skill development training or recreational coordination would be considered an asset
  • Experience with staff supervision and support or program coordination is considered an asset
  • Be on an afterhours rotation 23 times / year
  • Successful completion of CPR and First Aid and Police Vulnerable Reference Check (PVSC)
  • Valid driver's license with regular access to a reliable vehicle with $2 million liability insurance
  • Crisis Intervention and/or A.S.I.S.T. training would be an advantage.
