DUTIES AND RESPONSIBILITIES:


Working collaboratively with the multidisciplinary team, the Social Worker will participate in intake, pre-entry services as well as psychotherapy/counselling services with client seeking support from our Eating Disorders Program.

The social worker will provide treatment that focused on the delivery of high-quality evidence-based models that ensure care optimizes patients' recovery while supporting their parents and caregivers.

This position will require close collaborative care with the Mental Health and Addictions Central Connect Coordinated Access Team, referring physicians, nurse practitioners and other mental health professionals, both inside the hospital and in the community is essential.

This positon will involve completing intake calls, phone screenings, gathering supportive collateral referral information, case management of wait list, providing pre-entry services such as Orientation Group and Parenting Group, participating in all Eating Disorders Program clinical services, as well program development of pre-entry clinical services.

Identifying supports and resources available to the family is key activity for this position.

To perform other related duties as assigned.


QUALIFICATIONS:

  • Master's degree in Social Work
  • Registration (in good standing) with Ontario College of Social Workers and Social Service Workers
  • Membership in a registered professional association for practitioners in clinical therapy, is preferred;
  • Previous experience and training in individual and group psychotherapy with psychiatric population in an outpatient setting;
  • Established ability to provide intake and program matching services;
  • Training and demonstrated experience in evidence based practices such as FBT, CBT-E, Dialectical Behaviour Therapy and Cognitive Behaviour Therapy approaches;
  • Ability to provide information, assessment and entry facilitation including assessment, case management, clinical and supportive counseling, service coordination, crisis intervention, advocacy, and outreach.
  • Ability and experience with assessing and formulating a treatment plan for which is: creative; meaningful to the patients; workable within the time limits and includes an evaluation component;
  • Preference given to individuals with advanced training and experience in psychotherapy, e.g., process psychotherapy group skills
  • Demonstrated skill and experience in psychosocial and psychiatric assessment and intervention;
  • Ability to conduct Mental Health Assessments with excellent interviewing and communication skills;
  • Demonstrated knowledge of group theories and interpersonal relations theories;
- demonstrated ability in working with community agencies;
- proven ability to function within an interdisciplinary team;

  • Demonstrated knowledge of health care facilities and community support systems, services, social agencies, etc.;
  • Demonstrated ability to organize workload and establish priorities effectively;
  • Demonstrated ability to work independently;
  • Demonstrated skill in communicating effectively with patients, various institutions, agencies, and all levels of staff;
  • Proven to have strong initiative and problem solving abilities;
  • Proven to have effective written and verbal communication skills;
  • Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and
participation in regular in service education in this area;

  • Demonstrated ability to foster good public relations and effective working relationships;

  • Proven good attendance record with capability of maintaining this same standard.

The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and
OPSEULocal 348.
