Located in the community of Ponoka, within the parkland region of central Alberta, the Centennial Centre is one of the most modern mental health referral centers in Western Canada.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families
Your services may include:
psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:
Demonstrated ability to be informed by and work within applicable Alberta Legislations; Mental health Act (MHA), Alberta Guardianship and Trusteeship Act (AGTA), Personal Directives Act (PDA), Health Professions Act and Health Information Act.
Preferred Qualifications:
Experience working within complex care and strong system advocacy skills.
