Jean Tweed is a leading, community-based organization that provides a supportive environment for women with substance use, mental health, and gambling concerns.

Recognizing the diversity of women's experience in today's society, our programs seek to address a range of issues in a woman's life that may contribute to substance use and gambling concerns.

The Jean Tweed Centre is building capacity to transform the level of support for women with substance use, mental health, and gambling concerns._


The Bathurst Housing for Women and Children offers affordable, permanent housing to 10 women-led families with up to 2 children in their care.

This program is for women who are homeless or marginally housed, and who also identify substance use and/or mental health concerns.

They will be supported by case managers who will provide intensive support on issues related to substance use and they connect women with other important supports such as primary health care.

Women may also take part in a variety of groups offered by the case managers.


How will you help to get us there?
As the Case Manager for Bathurst Housing, you will:

  • Lead a collaborative wrap-around process to ensure comprehensive service coordination that reflects individual needs.
  • Provide case management support including counselling, outreach, ongoing assessment, goal planning, referral, development of support systems, system navigation, etc.
  • Provide referrals and support to access health and communitybased services.
  • Complete required case notes, evaluations, and data entry reports in an accurate and timely manner.
  • Conduct job responsibilities with a commitment to harm reduction, trauma
- informed, women-centred and anti-oppressive practices.

This role reports to the Director of Housing.


What will you bring?

  • Bachelors Degree in Social Work, or a related discipline.
  • A combination of 3 years of work experience in the field of Mental Health, Substance Use, and/or related field.
  • Expertise in providing counselling and case management to individuals who are experiencing or have experienced a complex history of mental health concerns, substance use, and/or homelessness.
  • Working knowledge of best practices in mental health, substance use, harm reduction, trauma and the therapeutic relationship.
  • Working knowledge of traumainformed practice, women centred principles and best practices for women and substance use.
  • Working knowledge of housing, health care and community resources in the Toronto area and related provincial resources.
  • Ability to work independently with mínimal daytoday supervision.
  • Crisis intervention skills, with welldeveloped problemsolving abilities and interpersonal skills.
  • Upon commencement of employment, all new employees are required to be up to date with vaccinations including the recommended doses of the Covid
  • 19 Vaccine as recommended by public health. You will be required to show proof of vaccination status during your orientation.
