Afterhours Community Care Supervisor
Full time position in our Edmonton Office

- Ability to work effectively as a member of a high performing team maintaining a positive working relationship with colleagues, team members, clients and their families and the referring agencies.

- Ability to work with a diverse population and adjust as needed to ensure positive outcomes
Essential Functions
- Works collaboratively with the Clinical Team to ensure the Community Care Staff understand their role and are confident and knowledgeable in individual patient care. As well as, ensures that the care provided and is completed to meet the individual care needs.
- Works effectively with the Clinical Team and the clients to ensure current care plans are in place
- Requests clinical field supervisions as necessary for assigned tasks and communicates effectively with the clinical team and our clients with respect to community client service provisions
- Managing requests for assigned task and the assignment of clinical supervision
- Maintain knowledge of community employee's work preferences, availability and competencies
- Maintain appropriate client and community employee's files where required Documents professionally, appropriately, and in a timely manner
QualifiationsCurrent registration with CLPNA
- Previous experience related to community care preferred (2 years minimum as an LPN)
- Proficiency in Microsoft Office Suite, and the ability to learn new programs specific to the business
- Previous leadership experience preffered.
