The Clinical Practice Lead is responsible for the effective and efficient delivery and sustainability of Programs and Quality within the Long-Term Care, Supportive Living, Hospice and Mental Health setting.

Implementation and sharing of policies/programs to ensure the highest standard is met as it pertains to policies and practices, and those set by the applicable legislation.

The Practice Lead plays a key role in the development and implementation of practice initiatives designed to support quality improvement and evidence-informed care for residents.

What You Will Do:
Training and Education

  • Support the professional practice of the Home Leadership Teams as applicable.
  • Assist in the implementation of new programs & in the revision and update of programs.
  • Provide innovative suggestions to assist with the overall implementation of new concepts to minimize the potential negative perception of change.
  • Orient, coach & mentor new managers/leaders; provide professional consultation to said leaders for their orientation, mentoring, and coaching for all roles.
Compliance

  • Promote compliance with regulatory requirements and standards.
  • Ensure regional/home involvement with, and representation on, applicable regulatory committees.
  • Ensure followup of highrisk issues.
Continuous Quality Improvement (CQI)

  • Ensure CQI & Accreditation programs are managed in accordance with standards, policies & practices, and in compliance with legislative and regulatory requirements.
  • Effectively represent the interests of AgeCare in all interactions with major external stakeholders.
  • Facilitate quality improvement framework through program review, audits, and data analysis.
  • Monitor and analyze performance indicators and adverse events.
  • Augment a culture of innovative thinking to generate ideas with the goal of improving practices.

Other duties:

  • Liaise with regional representatives.
  • Demonstrate ability to be flexible and adaptable to the changing needs of the business, including requirement for travel on short notice.
  • Participate in the oncall rotation as directed by the Regional Director.

What You Bring:

  • Preferred qualification: Degree in Nursing with continuing education in gerontology, nursing administration, rehabilitation, long term care or equivalent preferred.
  • Registered Nurse with current registration with the Provincial licensing body.
  • Current CPR certification.
  • Years experience: Minimum 5 years in a Long-Term Care, Supportive Living, Hospice and Mental Health setting at the senior management level an asset.
  • Familiar with using data to drive decisions and skilled in translating policies into direct practice.
  • Experience in gerontology/continuous quality improvement/research/adult learning/change management.
  • Strong knowledge of RAI-MDS preferred.
  • Demonstrates strong leadership, oral and written communication, and interpersonal skills.
  • Ability to facilitate learning for others.
  • Ability to reason, think analytically and conceptually prior to taking decisive action.
  • Proficient in computer skills, specifically in Word, Excel, and Outlook.
  • Must be able to provide a complete and current (within six months) Vulnerable Sector Check, including a Criminal Background Check, or be willing to obtain one.
