  • In cooperation with the Manager, Supervisor, and other YWCA Hamilton employees, the employee will perform all duties and responsibilities in a productive, professional manner while adhering to departmental and organizational policies / procedures and guidelines contributing to the overall goals of the department and the strategic vision of YWCA Hamilton.
  • In cooperation with the Program Supervisor this staff will perform all duties and responsibilities in a productive, professional and cooperative manner while adhering to departmental and organizational policies / procedures and guidelines which will contribute to the overall goals of the department and the strategic plan of YWCA Hamilton;
  • Ensure the physical and emotional wellbeing of individuals we support is maintained;
  • Meet QAM Compliance and MCSS Legislation 299/10 Standards or Schedule 20 By-Laws;

  • Learn how to provide direct and indirect support services support services may include, but are not limited to: casecoordination, advocacy, facilitation, administration, recreational planning, promotion of social inclusion, activities of daily living;
  • Support services will focus on enhancing the clients overall quality of life through meaningful activities and promote community participation and social inclusion;
  • Provide therapeutic recreation supports to individuals following Recreation Therapy Standards of Practice within Developmental Services Policy and Procedures;
  • Learn about

Case Coordination:
facilitating the linkage of support services / referrals / advocacy in order to meet individuals' needs in cooperation with individuals, families, other staff and community service agencies within the service plan domains;

  • Establish positive professional rapports with staff, clients, client's families or advocates, and other community service agency providers;
  • Provide assistance to participants in opportunities for community participation, socialization, recreation, and programming;
  • Observe and assist with the administration duties as required and as assigned ensuring written records / documentation and accountability are completed and maintained;
  • Observe the completion of accurate and timely reports as required; ensuring all Ministry and other reporting requirements are met;
  • Assist with the completion of intakes, assessments, and discharge summaries;
  • Observe and assist in the development of (ISP) Individual Support Plans and ISP Reviews and provide assistance in facilitating intervention strategies;
  • Plan and facilitate recreational activities, day program plans, groups and workshops as assigned;
  • Research information as assigned and complete reports on the research;
  • Participate in regular team meetings as requested;
  • Participate in training offered and attend ongoing professional development opportunities as assigned;
  • Learn and gain knowledge of individual program planning, personal development plans (PDP's), behavioural support plans (BSP's), protocols, and professional recommendations;
  • Implementation of effective engagement strategies, and Evidenced Based Practice, as appropriate, including and not limited to including TIP Core Practices;
  • Complete cleaning to ensure that housekeeping / cleanliness / maintenance of program areas are met;
  • Ensure YWCA Hamilton workplace health and safety policies and procedures are followed at all times and act to reduce workplace hazards. Report all workplace hazards and follow YWCA Hamilton health and safety policies;
  • Other duties as assigned.

Please Note:
The physical demands of the role may include, but are not limited to lifting (up to 25 kg), bending, twisting, reaching, sitting or standing for extended periods, walking, pushing, pulling, and moving constantly throughout the shift


Qualifications

  • Minimum Education Supported Independent Living:

Current Social Service Worker Student:
Diploma Program or Current Student in related Human Services Field;

  • Minimum Education Community Participation Day Programs:

Current Therapeutic Recreation Student:
Diploma Program,

Current Art Therapy Student:
Diploma Program;
