Your Opportunity:


Reporting to the Care Manager of the AMH Housing Team and as a member of a multidisciplinary team the incumbent will be responsible for collaborative intake and the provision housing and recovery supports in a new highly supported, harm reduction transitional housing initiative as well as other AMH housing initiatives.

The incumbent will work closely with referral sources, the housing provider, team members and other clinicians in reviewing referrals and supporting individuals based on a comprehensive strength-based assessment, intervention and evaluation that maintains the uniqueness of each individual within a collaborative wellness-oriented harm reduction service delivery model.

Essential to the skill set of this position will be a comprehensive awareness of housing resources, and an understanding of personal and systemic housing barriers, particularly with regards to individuals with significant concurrent addiction, mental health, and medical challenges.

psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).


Additional Required Qualifications:

You should be flexible, innovative, observant, and enthusiastic, with good communication and self-management skills in an interdisciplinary team environment. Current CPR (BLS Level C - Health Care Provider) is required. Experience in community capacity building, placement, and community development specific to this population and working with multiple stakeholders. Proficiency with Microsoft office, Excel and use of Connect Care and Strata Health Pathways.

Comprehensive awareness of a wide range of housing resources, knowledge of stakeholders, ability to build relationships, and an understanding of personal and systemic housing barriers, particularly with regards to individuals with significant mental health and/or concurrent disorder challenges.


Preferred Qualifications:

3-5 years' experience in community mental health settings, particularly with regards to housing assessment, intake, and support. Knowledge of and ability to respond to cultural needs of diverse populations is also an asset.

Developing and maintaining a welcoming and engaging environment and housing program culture for individuals with mental health and/or addictions and their family and/or primary supports.
