Position Overview:

Reporting to the Patient Care Manager of Social Work, you will be responsible for a diverse patient population. Social workers provide service across many programs including general medicine, surgery, emergency, and oncology.

Requirements:


  • MSW degree and registered with the Ontario College of Social Workers and Social Service Workers
  • Experience in a hospital setting
  • Excellent crisis intervention skills
  • Strong focus on psychosocial assessments and discharge planning
  • Excellent assessment skills and familiarity with community services
  • Strong ability and interest in linking patients to community resources
  • Must be flexible and a strong team player
  • Demonstrates excellent interpersonal and communication skills (verbal & written) and good judgement when relating to patients, families, coworkers and interdisciplinary teams
  • Demonstrates values consistent with SHN's Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
  • Utilize protective equipment provided and identify and reports hazards
  • Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
  • Excellent work performance and attendance record

Asset:


  • Expertise in supportive counseling
  • Ability to speak another language
