- Safe nursing practice in all aspects of the nursing process: assessment, planning, implementation and evaluation of patient care

- Master's degree in nursing and current extended class registration RN (EC) required with CNO
- Demonstrated patient/family teaching and staff education
- Exemplary clinical skills, knowledge and expertise
- Recent and relevant experience practicing as a NP with geriatric experience mandatory
- Seniors focus education including but not limited to gentle persuasive approach and PIECES courses
- Proficiency in computer skills and data management required (e.g. Microsoft Excel, Word and Access)
- Excellent organization, communication and interpersonal skills
- Excellent critical thinking, problem solving and organizational abilities
- Demonstrated ability to deal effectively with sensitive issues with families and staff
- Demonstrated knowledge and/or experience working within a professional practice and patient focused care environment
- Evidence of initiative in ongoing knowledge development and reflective practice
- Fluent in English (comprehension, reading, writing, and speaking)
- Ability to act as a change agent
- Current registration, and in good standing, with the College of Nurses of Ontario
- Sound knowledge of nursing process, theory and responsibilities to the nursing profession
- BCLS/ACLS required
- Demonstrated excellent interpersonal, communication, problem-solving, conflict-resolution and decision-making skills required
- Demonstrated ability to work collaboratively as a member of an inter-professional team
- Demonstrated knowledge of patient and family-centred care
- Demonstrated experience with patient and family health teaching required
- Demonstrated leadership and facilitation skills required
- Demonstrated excellent customer service, organizational and critical-thinking skills required
- Demonstrated commitment to continuous learning and professional development as it applies to the profession and related field
- Demonstrated ability to work in a stressful environment

- Ability to prioritize patient care needs and make autonomous decisions
- Excellent physical assessment skills required
- Ability to promote a healthy work environment and practice excellent client safety guidelines
- Knowledge and understanding of the Occupational Health and Safety Act

Description
Job DescriptionAs a member of the geriatric division and reporting to the Director, Women's and Children's and Seniors' Program, the Geriatric Nurse Practitioner (NP) enables the geriatric service to provide timely access to geriatric consults both on the Acute Care of the Elderly (ACE) unit and corporately. The seniors inpatient NP will demonstrate a high level of autonomy and expertise engaging in direct clinical practice in formulating clinical decisions and re-evaluating the patient's plan of care and focusing on the management of geriatric syndromes associated with the ACE Unit and complex dementia populations.
The principle responsibilities will include utilizing and demonstrating a comprehensive, advanced level of clinical knowledge and competencies, collecting and interpreting medical information about the health of the patient. The NP will work to their full scope of practice in very busy, dynamic acute care setting, contribute to improved outcomes, and will liaise and strengthen relationships with community and long-term care partners. A key area of focus for the NP is in enhancing partnerships with our long-term care homes for repatriation or discharge opportunities of our inpatient seniors' population.
The successful candidate will play a leadership role and perform quality improvement by participating in policy and procedure development activities/programs; acting as a resource and serving as a consultant to individuals and groups within the professional community and other hospital/agencies. The NP will facilitate educational support to patients, families and staff and will be a critical member of the clinical leaderships team supporting our Butterfly model of care on the ACE Unit. The NP role involves close collaboration with stakeholders to facilitate change and promote staff and patient/family satisfaction within a philosophy of patient-centered care.
