Human Services Counsellor - Toronto, Canada - St. Michael's Hospital
Description
As a member of our interprofessional Withdrawal Management Services team, the Human Services Counsellor plays an important role in supporting people who use drugs and/or alcohol who have expressed interest in temporarily abstaining from using and are seeking residential withdrawal management services at our 135 Sherbourne location.
DUTIES & RESPONSIBILITIES:
- Develops therapeutic relationships with people who use drugs and/or alcohol who have a goal to withdraw safely from substances
- Completes new client intakes, program transfers and discharges
- Maintains therapeutic milieu for residential WMS clients throughout their stay in the program
- Completes assessments of mental status, suicide risk and substance use
- Monitors clients for signs and symptoms of overdose, intoxication and withdrawal
- Develops collaborative care plans with community services and supports
- Engages in treatment planning activities including making inquiries about available community resources, completing referral forms, r monitoring service delivery
- Providing individual motivational interviewing counselling and psychoeducation around substance use
- Cofacilitating evidenceinformed group interventions including seeking safety,
- Provides crisis prevention and intervention as required
- Engages in transfer of accountability/shift report reports
- Completes clinical documentation
QUALIFICATIONS:
- Successful completion of college diploma program in Human Service Addiction Studies, Social Service Worker, Mental Health studies required
- Successful completion of university degree in arts or sciences considered an asset
- Successfully completed post graduate certificate in addictions and mental health considered an asset
- Previous experience working in an addiction counselling role considered an asset
- Excellent knowledge of best practices in addictions care and treatment required
- Excellent knowledge of community resources for people who use drugs and/or alcohol required
- Excellent therapeutic relationship and collaborative care planning skills required
- Excellent interprofessional teamworking collaboration, problem solving and conflict management skills required
- Very good understanding of the social determinants of health, antioppressive and trauma informed practice required
- Proficiency in using Microsoft suite and computerized clinical documentation systems required
HOURS:
- Must be willing to work 12 hour day and evening shifts on a rotational basis
