About the Opportunity:


Reporting to the Health Services Manager, Cumberland Primary Health Care, the Emotional Wellness Counsellor/Navigator (EWCN) demonstrates flexibility and innovation in facilitating health and wellness programs and in supporting citizens of the communities served by the team in accessing the programs and/or resources that support health enhancing behaviors.


Responsibilities of the Emotional Wellness Counsellor/Navigator include:

  • meeting with individuals and families with complex health and wellness needs to support them to prioritize needs, navigate health care and community resources using a social determinants of health lens.
- counselling individuals and groups with mild to moderate emotional distress using a Solution-Focused Approach and providing brief interventions to promote self-management of emotional wellness, overall well-being and to improve quality of life
- developing and facilitating group-based programs to address emotional wellness needs from both overall all wellness perspective as well as customized for populations with chronic conditions.
- researching, developing, and maintaining in depth awareness of resources within the community and the surrounding areas of the district through a community development, population health and collaborative approach.
- developing community partnerships and conducting community engagement, in collaboration with other multi-disciplinary team members.
- the EWCN initiates and follows through with emotional wellness interventions, providing seamless/ integrated service between Cumberland Community Mental Health and Addictions and across Cumberland Primary Health Care, Chronic Disease and Wellness programs and services.

This position will require significant travel.

The position will help address and build capacity to support our response to the Mass Casualty Commission's final report recommendations, relevant to Primary Health Care


About You:


  • Bachelor's (+ 5 years' experience) or Master's degree ( + 3 years' experience) in one of the following health professions is required: Social Work or MEd in Counseling
  • Master's degree preferred
  • Registration with and/or eligible to register with respective professional regulatory association required
  • 35 years related experience in: mental health; primary health care/community setting
  • 35 years related experience in: individual counselling; group education/facilitation using selfmanagement/behaviour change principles
  • Experience working with a lifespan population an asset
  • Preferred experience in partnership development, stakeholder collaboration, and community development
  • Experience working with people living with a complex and/or multiple chronic conditions
  • Understanding of population health, health promotion strategies, and behaviour change principles
  • Indepth knowledge of local health network and community based services
  • Demonstrates personcentered / familyfocused, diversity relevant approaches to care
  • Well developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility and adaptability to change
  • Demonstrates innovation in program planning, implementation, evaluation, and quality improvement
  • Planning skills

About You Continued:


  • Annual CPR-AED certification and annual WHMIS training and certification
  • Access to a reliable vehicle for work travel
  • Competencies in other languages an asset, French preferred

Compensation and Incentives:

$33.79- $42.63 Hourly
