Job Summary:
The Adult Day Program is focused on offering older adults a wide range of social, cognitive, physical and spiritual activities.
Responsibilities include but are not limited to:
- Provide full range of clinical social work services (e.g. psychosocial assessments, case management, clinical triage, crisis intervention, waitlist management & intake, individual and family counseling, future planning and education for members and their families)
- Facilitate virtual/inperson groups for members
- Facilitate virtual/inperson caregiver support groups
- Provide information and make referrals on a range of support services offered at Baycrest, within the Community & Province, offered online and inperson
- Provide education and assistance for members and families regarding POA's for financial and personal care, and advanced care planning
- Maintain accurate, up to date clinical documentation and proactively provide direct member status updates to members of the clinical team as appropriate
Qualifications include but are not limited to:
- Masters of Social Work degree required
- Current and valid registration with the Ontario College of Social Workers and Social Service Workers ('OCSWSSW') in good standing required
- At least one (1) year experience working in an Adult Day Program setting; experience working with older adults is preferred
- Experience with identifying and using a range of clinical assessment tools e.g. MMSE, MOCA, Suicide Risk Assessment, Depression Scales
- Experience working with people (Adults & Seniors) living with dementia, cognitive impairment, mental health issues, and families/caregivers across clinical settings, and able to provide the spectrum of supports (i.e. prevention, crisis, selfmanagement, future planning, etc.) and a comprehensive understanding of the diverse needs of members & caregivers
- Expert knowledge of Ontario LTC, Hospital & Community care environments, pertinent Health Services legislation (i.e. Regulated Health Professionals Act, Personal Health Information Protection Act, Long-Term Care Act, Public Hospitals Act, MAID), Clinical Ethics, Ontario Senior Support Services
- Excellent interpersonal communication, solution oriented & problemsolving skills
- Proficient computer skills & recent experience with Outlook, Meditech, Word, Excel, Resource Matching and Referral (RM&R) MDS required and OTN/ZOOM Virtual Care delivery preferred
- Experience with My Chart, OTN, Virtual Care preferred
