About the Opportunity:


The NSH Care Coordination Centre (C3), located in the central zone, provides coordination and integration of access and flow across our community, regional and highly specialized health services to improve the patient experience and the efficient and effective use of health care resources.

Though based in the Central Zone, the C3 plays a significant provincial and interprovincial role in the integration and coordination of health services to Nova Scotians and Atlantic Canadians.


Reporting to the Director of Access and Flow within C3, the Manager is responsible for providing operational leadership to multi-disciplinary teams within a patient-centred, collaborative practice model for patient care units and to manage the efficient use of patient access and flow resources integrated and coordinated within C3.

A responsibility to lead and engage stakeholders in the development, implementation and evaluation of improvement strategies to ensure efficient and effective utilization of resources.


Working collaboratively and consultatively, with patients/clients, families, management, staff, physicians, physicians' offices, community partners, and service providers, the Manager applies critical thinking to address challenges to achieve the ultimate goal of high quality, safe and cost-effective services to better support the patient experience/outcome, and to better support patient access and flow.


About You:

We would love to hear from you if you have the following:

  • Baccalaureate degree in nursing or equivalent experience
  • Minimum 5 years recent clinical experience in nursing
  • Minimum 5 years recent experience in a leadership and/or management position with responsibilities related to program planning, evaluation, human resource and financial management within a unionized environment.
  • Excellent oral and written communication skills
  • Intermediate level computer skills (MSOffice; Patient Flow Dashboard; Patient Audit System; Business Objects Patient Flow Reporting and SharePoint).
  • Experience with bed management software an asset
  • Strong understanding of bed management, utilization and patient flow methodologies as they relate throughout the continuum of care
  • Excellent conflict resolution skills to investigate and resolve problems
  • Demonstrated evidence of innovative leadership and critical thinking, with proven ability to foster a collaborative team approach to decision making
  • Understanding of continuous quality improvement tools and methodologies as it relates to system thinking as it involves public/culture, staff as clients, staff as employees, physicians, other government sectors, contracted services, etc.
  • Strong process knowledge and process thinking abilities
  • Ability to work collaboratively with and engage others within an interprofessional practice environment
  • Competencies in other languages an asset, French preferred

Compensation and Incentives:

$ $56.64 Hourly
