$ $56.37 (start rate in accordance with collective agreement)

As a member of the interdisciplinary client care team, the CARE LEADER collaborates with other members of the team to ensure the effective management of client outcomes.


The primary responsibilities of the CARE LEADER include:
providing a clinical leadership and resource role within the nursing team; coordinating client care management processes such as home care coordination; providing direct patient care through:
Assessment - systematically collecting data about the client for initial peritoneal dialysis treatment, on-going self or assisted care.

Planning - identifying priorities, target dates, strategies or interventions in order to contribute to the individualized care plan that achieves optimal outcome for the client.

Implementation - safely and effectively performing the strategies and interventions directed by the care plan.

Evaluation - systematically evaluating the extent to which the best possible outcomes have been achieved, the overall nursing practice and system for delivering care including care provision within the community; documenting client records; continually acquiring and upgrading knowledge and skills; providing educational guidance to client population, students and care team members; participating in related research activities; acting as a client advocate by protecting and promoting client rights; assuming other "Charge" responsibilities as assigned; performing other duties consistent with the job classification, as required.


Qualifications

  • Completion of BSc.
N., Masters preferred

  • Post graduate certificate in Nephrology Nursing or CNeph C required.
  • Post graduate certificate in Adult Education and or Leadership preferred.
  • 58+ years Peritoneal Dialysis and Hemodialysis experience required.
  • Current certificate of competency from the College of Nurses of Ontario required
  • Basic Cardiac Life Support (B.C.L.S.)

program, Level II:
Basic Rescuer certification

  • Evidence of continuing education, committee and or professional organization involvement and preceptorship involvement preferred.
  • Demonstrated leadership abilities required
  • Excellent time management skills required
  • Excellent interpersonal skills required
  • Excellent clinical decisionmaking and problem solving skills required
  • Excellent verbal and written communication skills, including both active listening to patient, families and team members
  • Clinical and client teaching skills required
