_ Program Overview - Crisis Outreach Service for Seniors, Community Care_

  • This position works on behalf of WoodGreen in an interagency, multidisciplinary Crisis Outreach Service for Seniors (COSS) serving seniors with physical, cognitive, mental health, and other concerns causing them to be in crisis. This service is offered through a partnership between WoodGreen, Haven Toronto, South Riverdale Community Health Centre, Reconnect Community Health Services, and LOFT Community Services with the goal of ensuring a continuum of care for seniors with cognitive, mental health, and substance use issues._

What You Will Do_**- _
Participate in crisis shifts; visit clients in their homes or community locations with a partner._
- _ Perform psychosocial and geriatric assessments. _
- _Provide preliminary assessment of risk in crisis situations, consult and devise an intervention and action plan, and provide crisis intervention. _
- _Provide crisis intervention and practical assistance to clients and/or family or caregivers as required in homes and community settings._
- _ Develop and implement service plan and/or coordinated care plans._
- _ Provide service/care co-ordination, system navigation, short and long-term planning and follow-up support._
- _ Provide crisis stabilization and short-term case management services to clients through the development and implementation of a care plan to address their priority and emerging needs. _
- _Provide supportive counseling to clients utilizing a wide range of therapeutic approaches that best meet the clients' needs and circumstances._
- _ Participate in case conferences and clinical meetings in order to ensure the coordination of services and a continuum of care._
- _ Provide advocacy support and advocate with clients and/or their family or caregivers._
- _ Provide Transitional Care Coordination and work closely with the hospitals, and other referral sources to support clients and facilitate smooth transitions between hospital and community. _
- _Link clients and/or their family or caregivers to various community resources, and facilitates appropriate referrals to other community support services._
- _ Provide service that is responsive, culturally and linguistically sensitive. _
- _Engage in ongoing outreach to ensure access to the program._
- _ Collaborate with community agencies to organize joint health promotion clinics/workshops._
- _ Maintain and develop contacts with other seniors' groups and community agencies, and attend external meetings as recommended by the Manager and/or Unit Director._
- _ Keep current about the range of services available in other agencies. _
- _Provide mentorship, training and/or supervision to student placements and volunteers._
- _ Maintain a client caseload as assigned by program manager._
- _ Promote the healthy functioning of the team and unit and participate in meetings regularly._
- _ Participate in ongoing training and education._
- _ Participate in supervision sessions and annual performance appraisals with program manager._
- _ Maintain appropriate records and documentation in a timely manner and collect statistical data required for the accountability of the program._
- _ Responsible for representing the unit on internal staff committees as directed by program manager or unit director._
- _ Ensure the anti-racism objectives and community development principles of the organization are achieved within the program._

What You Bring to the Team_**- _
Masters of Social Work or post-graduate degree in relevant field. _
- _Registration in good standing with the Ontario College of Social workers and Social Service Workers or another regulatory body._
- _ 3+ years of experience in mental health, addictions, geriatrics and/or social work._
- _ Experience working with older adults and seniors with mental health and/or substance use issues, chronic health issues, cognitive impairment, and responsive behaviours._
- _ Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption)._
