Job Summary:Baycrest has been appointed as the health service provider accountable for leading the implementation of the Toronto Central regions' Behaviour Support Action Plan. A key component of the plan is the deployment of a Behaviour Support Outreach Team ('BSOT'), consisting of interdisciplinary community outreach clinicians, nurses and personal support workers who have behavioural support expertise. The Personal Support Worker is a key member of the direct care team who works with the same group of clients. Their functions are directed by input from other members of the team and a written care plan.

The PSW
_ _promotes the clients' quality of life and assists them to maximize their full potential by:

providing care to meet the client needs as identified in the interdisciplinary care plan; supporting the client's independence, dignity, social, emotional and cultural well-being, and; initiating and maintaining a relationship with the client, their family and other care providers.

The PSW provides support to clients living in the community and long term care sectors with an emphasis on transitional support, observation tasks and modeling behavioural strategies to the circle of care.


Responsibilities include but are not limited to:

  • Supports, in consultation with the other clinical team members, the physical and personal care needs of the client in context of the plan of care
  • Uses professional judgment, monitors and evaluates client care needs and care goals regularly according to standards, policies, procedures and legislation by analyzing and interpreting overall client outcomes and participating in modifying Care Plans as neededParticipates in required transfer of accountability reports at the start and end of shift
  • Supports transitions across sectors
  • Coaches the client's circle of care (formal and informal) nonpharmacological behavioural recommendations
  • Completes client observations to help identify biopsychosocial triggers for responsive behaviours
  • Completes daily log summaries at the end of each shift
  • Supports the social, emotional, cognitive and spiritual wellbeing of the client under the guidance of the direct care team
  • Maintains effective interpersonal relationships with the direct care team, clients, families and volunteers
  • Understands and utilizes key client safety concepts, i.e. adverse events, incidents and risks
  • Assists with transitional support, observations of behaviours, and modeling nonpharmacological interventions to care providers in the community and LTC settings across the Toronto Central region
  • Participates actively in the Orientation program
  • Attends education opportunities when not assigned to regular care tasks

Qualifications include but are not limited to:

  • Successful completion of a recognized Personal Support Worker program
  • Ontario Secondary School Diploma or recognized equivalent
  • Have current Basic Cardiac Life Support ('BCLS'), Level C
  • Recognized First Aid certificate
  • Certificate from the Alzheimer's Society
  • Certificate in Dementia Care is preferred
  • Certificate in Ufirst is preferred
  • Basic computer skills (word)
