Position Summary

The Social Worker as a key member of the multidisciplinary team, and provides treatment modalities that may include individual and family therapy, group work, consultation, case navigation, crisis intervention, community outreach and advocacy in a manner that is consistent with the Quinte Health values and that reflects the core values of Patient and Family Centred Care (dignity, respect, information sharing, participation, and collaboration); practices in accordance with the standards of professional practice and ethical values as defined by the Ontario College of Social Workers and Social Service Workers; participates in service and hospital-wide activities and programs that model and promote a safe and healthy patient-centered environment and that contribute to quality improvement and risk management initiatives.

Develops and coordinates treatment plan in conjunction with the treatment team including connection/referral to appropriate internal and/or community resources; this position is assigned to the Sills 5, Inpatient Mental Health Program.

Required

  • Master's Degree in Social Work
  • Eligible for membership in the Ontario College of Social Workers and Social Service Workers (OSWSSW).
  • Current enhanced security clearance (criminal reference check) for vulnerable populations administered by the Canadian Police Information Centre (CPIC)
  • 35 years' clinical work experience in a mental health setting working with a broad range of patient groups.
  • Demonstrated assessment and therapy skills with expertise in individual, couple, family and group work required.
  • Knowledge of relevant legislation and community resources.
  • Demonstrated ability to function independently and as part of a multidisciplinary team.
  • Superior communication skills required, oral and written.
  • Current First Aid and BCLS at the Health Care Provider level
  • Proven ability to attend work regularly.
Duties

  • Demonstrated knowledge of DSM-V diagnostic criteria for psychiatric disorders most prevalent with adults.
  • Highly developed clinical skills in interviewing, psychosocial and risk assessment, treatment (individual, group and family therapy) crisis intervention and cognitive behavior therapy (CBT).
  • Provides individual and group therapy sessions to patients and their families, instruct other mental health staff in therapeutic techniques, provide crisis interventions, and arranges for alternative community services when needed; training in CBT, dialectical behavior therapy (DBT) and solutionfocused brief therapy is preferred.
  • Demonstrated knowledge of the Ontario Mental Health Act, Health Care Consent Act, Powers of Attorney Act, Substitute Decisions Act, Personal Health Information Act and other relevant legislation.
  • Is accountable for decisions and actions when acting independently or when acting in consultation and collaboration with other members of the program team.
  • Creating treatment plans with patients and their families, and maintaining patient records in accordance with privacy legislation while completing assessments, preparing reports, monitoring progress toward treatment goals and conducting regular reviews of active treatment plans.
  • Extensive knowledge of evidence based practice and current experience in the assessment and treatment of complex mental health disorders.
  • Knowledge of assessment and treatment for substance use, addiction and concurrent disorders is an asset.
  • Facilitates the integration of evidencebased best practices into professional services.
  • Strong communication skills and confidence in social work scope of practice and articulating this within a health care setting.
  • Demonstrates sensitivity and awareness of the stigma frequently experienced by persons carrying a diagnosis of a psychiatric disorder.
  • Demonstrated ability to work as part of a multidisciplinary team of psychiatrists, allied health and nursing professionals and external agency representatives
  • Highly developed communication, presentational & documentation as well as strong mentoring & coaching skills
  • Effective organizational & time management skills
  • Capable of coping in a physically & mentally demanding work environment
  • Proficiency with current versions of MS Office software
  • Recognizes and responds effectively to unexpected situations.
  • Maintains privacy, confidentiality, and the security of client, staff, and organizational data.
  • Initiates and maintains community liaison with other community agencies, groups, and service stakeholders.
  • Contributes to an environment of mutual respect and understanding.
