Join our new multi-disciplinary team of passionate and committed professionals working to support clients with complex mental health issues and/or substance use disorders, and have come into contact with the law.

This Vocational Rehabilitation Counsellor position will support the Enhanced Care Housing Project.

The Enhanced Care Housing Project supports clients found not criminally responsible or unfit to stand trial due to a mental disorder as they transition out of the Forensic Psychiatric Hospital and into the Community Transitional Care (CTC) housing program located on the səmiq̓wəʔelə lands (former Riverview Hospital Lands, Coquitlam).

The CTC program provides recovery and rehabilitation services to facilitate a safe and successful transition and reintegration of forensic patients into the community.

This position will be temporarily based out of the Surrey Forensic Regional Clinic; but will later transition permanently to the səmiq̓wəʔelə lands.

This role is ideally suited to a person who enjoys supporting clients to achieve success with reintegration in their community, working in a team environment, solving problems, and working collaboratively with partners.

This person must be committed to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC and DRIPA

What you'll do

Assess and evaluate patients' vocational rehabilitation needs on an ongoing basis through methods such as collaborating with the Case Management Coordinator, Treatment Team, Primary Nurse and the patient to determine interests, skills and abilities, encouraging patients' involvement in plan development and facilitating the setting of appropriate vocational rehabilitation goals in accordance with clients' preferences, needs and abilities.
Receive referrals and develop individual vocational rehabilitation plans to integrate vocational goals into the mental health treatment plan through methods such as engaging patients in discussion to determine individual needs and potential career paths, abilities and interests and building a trusting and respectful relationship; orienting patients to programs at the hospital, making recommendations for placement considering patients' expressed preferences/interests and suitability. Coordinates prevocational training for patients so that they are able to take full advantage of programs available.
Liaise with the Case Management Coordinator, collaborate with members of the interdisciplinary team and community professionals and advocate on behalf of the patient to assist them in accomplishing a goal of competitive employment or other viable vocational pursuits in an integrated community setting.
Monitor vocational training placements, track progress, evaluate programs, and participate in overall case planning for patients by methods such as visiting work/program areas on a regular basis and, in collaboration with the program staff, assessing patient progress, writing progress notes and reports; informing the treatment team of progress, any concerns, changes recommended regarding the patients' development in the program; and managing the patient incentive gratuity program and administering the Holiday Program.
Provide ongoing support services to clients related to employment through methods such as providing vocational rehabilitation counseling to clients on an individual basis by telephone or at the job site, facilitating group meetings and liaising with Case Management Coordinator and other members of the interdisciplinary care team. Provides onsite job coaching to ensure patient orientation and ongoing training as required.

What you bring

Education, Training and Experience

Bachelor's Degree in an applicable social or health sciences program and two years' recent, related experience providing vocational rehabilitation counselling or an equivalent combination of education, training and experience.
Class 5 Driver's License

Skills and Abilities

Demonstrated knowledge of psychosocial rehabilitation principles.
Demonstrated knowledge of mental illnesses.
Ability to assess, plan, organize, implement, evaluate and problemsolve.
Demonstrated ability to communicate effectively both verbally and in writing
Ability to work effectively with others, including working effectively in a multicultural environment, independently, as part of an interdisciplinary team and understanding the frameworks of other disciplines.
Demonstrated ability to deal effectively with and resolve conflicts.
