POSITION PURPOSE:
MAJOR RESPONSIBILITIES:
- Attends all LTC Homes RAIrelated education sessions.
- Participates in the planning and organizing of the Home RAI process change.
- Ensures a resident assessment schedule is developed, completed, current and accessible to all RAI users.
- Assists RAI users to correctly code assessments and provides support for validation of coding.
- Oversees the development and coordination of the data submission process.
- Generates RAI-MDS reports, other Quality Progress Reports and facilitates their use in clinical and operational decisionmaking.
- Completes and submits all required RAI assignments as required to the assigned lead.
- Works with RAI users to develop processes that ensure data accuracy, efficient data collection process, assessment completion and data utilization.
- Oversees the RAI process according to Home policies, MOHLTC regulations, College Standards of Practice and interRAI requirements (see CIHI User's Manual).
- Ensures MD assessments, resident assessment protocols and care planning are completed on a timely basis.
- Conducts audits of staff input to ensure accurate reporting and submissions.
- Monitors and provides onsite support to all RAI users in the Home.
- Communicates regularly with the RAI users and management team and with Ministry staff.
- Performs other duties as assigned, in accordance with Branch and Department objectives.
- May occasionally be required to respond to emergency situations outside of normal business hours, as related to the Departmental Emergency Plan.
QUALIFICATIONS
- Successful completion of a Community College Diploma in Health Sciences or related field or approved equivalent combination of education and experience.
- Current regulated or licensed health care practitioner (e.g. RPN).
- Minimum one (1) year in a Long Term Care setting with clinical experience including restorative care programming and using RAI-MDS, project leadership, and experience with adult education strategies/facilitation.
- Experience in working on continuous quality improvement project implementation.
- Demonstrates strong initiative, critical thinking and enthusiasm to collaboratively support the development of interdisciplinary care programs and other initiatives
- Satisfactory Police Vulnerable Sector Checks (PVSC).
- Knowledge of Long Term Care or health care policies on assessments and care planning processes.
- Knowledge and demonstrated ability in corporate core competencies including customer focus, communication, collaboration and personal ownership.
