Job Summary:

Identifying and coordinating internal and external resources for youth and their families during the youth's admission, as well as their transition to the community.

Duties & Responsibilities:

Case management to coordinate and support appropriate services for youth and their families while at Syl Apps.
Identifying, developing and coordinating appropriate discharge plans for youth by collaborating with internal and external stakeholders
Verbal communication and writing documentation that addresses youth's care and treatment. Coordinating meetings and appointments as mandated and/or as needed.

Qualifications:

Requires completion of University degree or equivalent.
Some practical or position related experience required.
Satisfactory Criminal Record Check (including a Vulnerable Sector Screening) and Pre
Employment Medical

Kinark Child and Family Services conditions of employment include but are not limited to successful Criminal Record Check with Vulnerable Sector Screening, completed Pre-Employment Medical certificate, and proof of full vaccination against COVID-19. Fully vaccinated is defined as having received all required doses of a World Health Organization Approved COVID-19 Vaccine at least one of which being approved for use in Canada and 14 days have passed since your last dose. Proof of vaccination for each vaccine dose received is required.
