System Navigator - Fetal Alcohol Spectrum Disorder (FASD)1.0 FTE

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking a 1 FTE System Navigator - Fetal Alcohol Spectrum Disorder (FASD) responsible for providing support for clients with complex and multiple needs referred to the FASD program.
The System Navigator supports clients and families with navigating health and social support systems and coordinates groups and programs that meet client needs.
The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package.
QUALIFICATIONS:
- Honours Bachelor Degree in Social Work (HBSW) or a combination of academic educational training and applicable experience.
- Knowledge and experience working with individuals diagnosed with Fetal Alcohol Spectrum Disorder
- Ability to effectively engage clients in the assessment and planning with regards to their needs.
- Demonstrated ability to facilitate and advocate for services to support clients in their care.
- Ability to work both independently and collaboratively with an interdisciplinary team.
- Strong writing and documentation skills.
- A strong understanding of the determinants of health framework.
- Strong interpersonal, organizational and group facilitation skills.
- Ability to work flexible hours.
- Understanding and experience working in diverse, lower socioeconomic status communities.
- Excellent organizational, oral and written communication skills.
- Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, noninsured).
- A vehicle and a valid driver's license are required.
