Patient Flow Coordinator

Temporary Full-Time (Contract until Spring 2024)

Hourly rate:
$ $41.88

Start date:
as soon as possible

QUALIFICATIONS:

Regulated Health Professional registered with the applicable professional college
Two to three years of experience in rehabilitative care environment
Knowledge of and experience in determining patient eligibility for rehabilitative care programs in collaboration with referring agencies
Excellent communications skills to interact with the interprofessional team and system partners
Strong interpersonal, organizational, and people management skills
COVID19 Vaccination mandatory (2 Doses)
