    DUTIES AND RESPONSIBILITIES:

    Working as a member of the inter-professional team of the Mental Health Program, the Nurse Practitioner (NP) will provide ongoing primary care services to adults with primary mental health disorders with a focus on assessing and treating physical health needs in collaboration with our Medical Team and Psychiatrists.


    The NP is responsible for:

    autonomously diagnosing, ordering and interpreting diagnostic tests; prescribing pharmaceuticals and performing specific procedures within the legislated scope of practice; , and providing direct patient care focusing on health promotion and maximizing clients' safety and function during their participation in program services; actively participating in regular team meetings and in discharge decisions, liaising with community based and hospital providers, advocating on behalf of clients and their families and participating in inter-professional patient and family conferences as needed.

    The successful candidate will also have opportunities to participate in professional development and continuing education to maintain clinical competence and to maintain optimal quality of care to clients


    QUALIFICATION:
    *Completion of a recognized Nurse Practitioner program

    • Primary Health Care Nurse Practitioner preferred
    • Current registration with the College of Nurses of Ontario in the Extended Class;
    • Working knowledge of the Mental Health Act of Ontario;
    • Training and experience in the area of general mental health and related prescribing preferred;
    • Demonstrated effective communication and interpersonal skills to work effectively on an inter-professional team and with community partners;
    • Effective organizational skills;
    • Demonstrated ability to exercise independent judgement and to assess situations and problems efficiently and effectively;
    • Proven strong consultation skills;
    • Committed to client-focused philosophy;
    • Must have satisfactory performance record;
    • Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;
    • Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area; and;
    • Proven good attendance record with capability of maintaining this same standard;

    CONDITIONS OF EMPLOYMENT:

    The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).

    Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
