About Us:

Reconnect Community Health Services is a not-for-profit health service organization in the west end of Toronto. Our services include supports for seniors, caregivers, and people living with mental health and addictions concerns. We pride ourselves on being at the forefront of client-centred care in our sector.


As a Community Development Worker you will plan, execute and evaluate activities and programs that maintain and promote the physical, emotional and cognitive well-being of the Health and Wellness Centre for Seniors clients.


What You'll Do:

You will be entrusted with a wide range of responsibilities, including:

  • Collaborate and assist with the implementation of special projects, summer activities and special events;
  • Monitor and assess client's physical, mental and emotional wellbeing and progress and adapting programs to meet their needs;
  • Research, report and recommend new and updated recreational, social and motivational activities for the Health and Wellness Centre for Seniors programs that promote and support through facilitation, outreach and making connections, the physical, emotional and social wellbeing of clients;
  • Providing support and direction to program volunteers and students.

What We're Looking For:


  • Activation Diploma or related education is required;
  • Experience working with older adults in a formal program setting.
  • Strong organization skills and the ability to multitask;
  • Team development and leadership skills;
  • Excellent communication skills, both verbal and written;
  • Applied computer skills (Microsoft Office, Client Information Systems, Web browsing);
  • Fluency in a second language is a strong asset and preferred.

Please be advised that in order to be eligible for employment at Reconnect, all new hires must have received both doses of a COVID-19 vaccine approved by Health Canada prior to their start date and will be required to show proof.

$21.86-$23.73 per hour

Experience:


  • Working with older adults in a formal program setting: 2 years (required)
  • Planning and executing activities and programs: 2 years (preferred)

Licence/Certification:

  • Activation/Recreationist diploma (required)
