Job Summary

The Manager, Patient Care Transitions is a member of the Patient Flow and Access team and is responsible for the overall day-to-day coordination of patient and family transitions, facilitation of patient flow, preparing and submitting required reports within established timelines, and is responsible for the ongoing development, implementation and continuous quality improvement in the discharge planning and Alternate Level of Care (ALC) processes.

They will work with the patient flow team, the discharge planning teams, individual units/programs, and clinical leaders to support Patient Transitions and ALC management.

This leader will be required to work as a collaborative team member developing frameworks and methodologies of healthcare performance leading to a superior ease of flow and operations within a challenging healthcare environment with diverse programs and multiple competing priorities.

  • Experience with ALC patients, practices, and escalation pathway;

Qualifications:


  • Bachelors Degree in a health related discipline, Masters Preferred
  • Minimum of five years recent and relevant clinical experience, demonstrated excellence in clinical practice and is academically prepared and experienced in the area of Acute Care.
  • 2 years experience in Discharge Planning and the coordination of ALC patient care, responsible for patient flow, expert knowledge of discharge planning process.
  • 3 years experience in an administrative or leadership position
  • Nursing knowledge gained through formal education and registration as a Registered Nurse with the College of Nurses.
